Service Prices:
45-Minute Initial Consultation: $500
30-Minute Strategy Session: $250
Cancellation Terms:
​No show or cancellation within 24 hours results in a fee of $150
Please notify for cancellation or rescheduling at least 48 hours in advance
Payment & Insurance:
​Payment is due at the time of service. Healthspan accepts payments via credit card, Venmo, and HSA/FSA, but does not accept insurance.

Scheduling an Appointment:
All Healthspan appointments are scheduled on Tuesday's and Wednesday’s between 8:00am - 2:00pm.

Please fill out the form below to make an appointment and include two preferred appointment dates.

You will be contacted within 48 hours to confirm an appointment date and time, or to find an alternative date if your preference dates are not available.

After your appointment is confirmed, you will receive an email link to activate your patient portal where you can fill out patient forms. All forms must be completed 24 hours before appointment time.
